Cannot create service escalation for services in host template

Using:
Lilac 1.0.3 (From webpage download)

Several services are common across different types of servers. I have created a host template that contains these service definitions.

When I try to add a Service Escalation for any of these 'templated' services, I get a blank page for the after clicking the "Create a new escalation for this template" button. The URL displayed by the browser is  http://localhost/lilac/add_escalation.php?service_id=300

The page source is completely blank for this page.
